The records division is open Monday through Friday 8:30 a.m. to 5:00 p.m. and closed holidays.
Requests will be processed as quickly as possible. In most cases citizens will be able to obtain a copy of their report in one visit to the records division. However, in some cases it may take ten business days or longer to receive a copy of the report depending upon the circumstances of the investigation. A records personnel will keep you updated of the status of your request.
To make a record request, please select an option to the right. For Bountiful Police reports other than traffic accidents, your request must be made through mail, email, fax, or in person. If you don’t want to use the online method to get a traffic accident report, you may still use one of the other methods.
Please remember, you must pick up the report in person and show photo identification.

Online Requests - Traffic Accidents Only
Visit crashreport.utah.gov and enter your information to access a copy of your report. A utah.gov account is not needed, use your personal email address in the email field.
Utah.gov provides an official DI-9 (the state regulated vehicle crash report) form in PDF format. Witness statements and additional supporting information will only be available from the reporting agency.
